after a dramatic flashforward revealed Caleb Miligan (William Ash) unconscious in a hospital bed, hooked up to life-saving machines. 

The shocking scene had viewers speculating about the real reason behind his hospitalisation 

and, surprisingly, it’s not linked to his confrontation with his brother Cain (Jeff Hordley), as many originally thought.
